This process involves stimulating and monitoring in a laboratory. For the next 2-6 days, the fertilized eggs undergo embryo culture also. Later on, it is implantation that takes place in the same or another woman’s uterus. This process intends to establish a successful pregnancy. IVF is a process appropriate for gestational surrogacy and infertility treatment.

  • Women with blocked fallopian tubes – The IVF procedure completely bypasses the fallopian tubes. If you are producing healthy eggs that are being blocked from fertilization (or if you have had your fallopian tubes removed), then you may be a right candidate for IVF
  • Women with an infertility disorder – Polycystic ovarian syndrome (PCOS) is the most common cause of infertility in women. If this is the reason for your problems, then IVF may be the right solution for you. IVF may well serve women with other conditions such as endometriosis.
  • Women with irregular ovulation cycles – IVF treatment produces more healthy eggs by inducing ovulation, countering an irregular menstrual cycle. 
  • Men with factor infertility – If your partner has a low sperm count, irregularly shaped sperm, or another male factor infertility challenge, IVF paired with ICSI can deliver a successful fertilization and conception. 

The IVF process includes five key steps:

  • Ovarian stimulation
  • Egg retrieval
  • Sperm collection/retrieval
  • Fertilization & embryo development
  • Embryo transfer

An IVF cycle normally lasts about a month, very similar to a regular reproductive cycle. Day 1 starts at the first day of menstrual bleeding. Most egg collections occur around the 12-14th day of an IVF cycle. But it may vary individually, from patient to patient, and from cycle to cycle.
On the day of the egg collection, the egg is exposed to sperm (IVF) or a single sperm is injected into the egg (ICSI). The embryo transfer will be performed 3-5 days later. And pregnancy test will be performed two weeks after the egg collection. Some clinics utilize oral contraceptives to control the period to ensure the patients going through treatment right on schedule.

Some males are more likely than others to experience infertility. You might be more likely if:

  • You’re overweight or obese.
  • You’re age 40 or older.
  • You’ve been exposed to radiation.
  • You’ve been exposed to environmental toxins including lead, calcium, pesticides or mercury.
  • You’re a tobacco, marijuana or alcohol abuser.
  • You’re taking some medications including cyproterone, flutamide, spironolactone, bicalutamide, cimetidine or ketoconazole.
  • You’re around heat that raises the temperature of your testes. Those who frequently use a sauna, hot tub or wheelchair might experience this.
  • You have a history of undescended testicle(s).
  • You have a history of varicoceles, which are widened veins in the scrotum.

The success rate depends on a number of factors including reproductive history, maternal age, the cause of infertility, and lifestyle factors. Infertility is the result of a disease of the male or female reproductive tract which prevents the conception of a child or the ability to carry a pregnancy to delivery. The duration of unprotected intercourse with failure to conceive should be about 12 months before an infertility evaluation is undertaken, unless medical history, age, or physical findings dictate earlier evaluation and treatment.   • Pregnancy chances depend heavily on your age, the causes of infertility, the techniques used, and the experience of the team looking after you. Globally, the average probability of getting pregnant after IVF is about 25–30 percent; best-practice clinics reach 35–40 percent and up to 65 percent in couples where the woman is under 32 years of age. Pregnancy rates are lowest for women over 40.

    About 15–20 percent of couples, or about half of all women over 40, suffer a miscarriage after IVF. The risk of miscarriage can be greatly reduced by a genetic examination of the embryo; however, depending on the country you’re being treated in, this exam is only permitted under certain conditions and can be very expensive.

IVF (in vitro fertilization) is intended to help couples who cannot get pregnant naturally. The most common reasons for IVF include

  • Reduced sperm quality and/or quantity
  • Blocked fallopian tubes
  • Polycystic ovary syndrome (PCOS)
  • Reduced ovarian reserve
  • Endometriosis

Natural cycle IVF is done without any hormone medication with the aim to collect naturally growing egg from the ovaries.

Advantage: It can be used for woman who want ‘Drug Free IVF’. It is cost effective in terms of medication cost

Drawback: Risk of natural ovulation inside body before oocyte retrieval. Low success rate as usually only 1 egg is collected. It has a very high chance of cancelled treatment due to no egg retrieved at time of OCR .

There are six to seven days during your menstrual cycle when you can get pregnant. This is known as your fertile period.

 

You are most fertile during the five days before you ovulate, the day that you ovulate, and possibly a day after you ovulate.

Sperm can live up to five days. Once released, an egg only lives for about 24 hours. This is why there are six to seven days that count as being within your most fertile time.

In modified natural cycle IVF minimum medication are given to help collection of naturally growing eggs from the ovaries.

Advantage: It can used for woman who wish to take minimum possible medication.

Drawback: Low success rate as usually only 1 or 2 oocytes are collected.

In Mild IVF low doses of fertility medicines are given for 5 to 10 days to promote growth & collection of 2-5 eggs.

Advantage: It can be used for women with low ovarian reserve, where ovaries can produce 2 to 5 eggs despite maximum those of fertility medication. So, choosing for mild IVF in such cases reduces cost of medication & also emotional burden of the treatment.

Drawback: Lesser success chance per started cycle as lesser number of oocytes & embryos are available.

Normally a matured oocyte called Metaphase 2 ( M2 ) oocyte is released from ovary at the time of ovulation. Similarly, during routine IVF procedure matured oocyte is collected from the ovaries.

In IVM process immature eggs ( Germinal vesicle & metaphase1 ) are collected from the ovaries & then matured inside incubator in embryology laboratory. This is why it is called In Vitro Maturation ( IVM ).

In Vitro Matured egg is then used for ICSI.

Advantage: It can be used for women with Polycystic Ovarian Disease to avoid risk of ovarian hyperstimulation. Women who want fertility preservation before starting treatment for cancer, but have very limited time ( only few days ) can opt for IVM of oocytes & IVF/ICSI.

Drawback : It has lesser success per started cycle as compared to conventional IVF. .

In IVF & ICSI both processes semen sample is processed to separate healthier fraction of sperms.

During IVF process sperms are incubated overnight in culture dishes with aspirated mature oocytes . Here fertilization takes place naturally.

ICSI is advanced modification of IVF.

During ICSI each oocyte is held & injected individually with selected sperm by trained embryologist . That is why the term ICSI- Intra Cytoplasmic (oocyte cytoplasm) Sperm Injection is used

ICSI is preferred over IVF in following cases:
  • Woman with low number of eggs.
  • Men with very low number of sperms.
  • Men with poor sperm quality.
  • Men with poor sperm motility.
  • Men with perm retrieved after TESA / micro TESA in cases of azoospermia.
  • Couple with previous partial or total fertilisation failure.
  • Couple using frozen sperms.

IUI or Intra Uterine Insemination is a process where the sperms are put inside the uterus after washing. The IUI is done when egg is going to be released or has just released from the ovary. Get IUI Treatment in Indore at Yaami Fertility and IVF Center and enjoy the feeling of parenthood.

There are four basic tests which can be done for a couple have any infertility. These include cement test, ultrasound to check for the ovulation and the uterus and the Jubilee patency test. If we test hundred couples, 60 of them will have an abnormality in the above four testings while the other 40 would have no abnormality. These 40 couples are said to have a condition known as “unexplained infertility” which basically related to problems with the  egg quality or the problems with the fertilization of the egg by the sperm.

Laparoscopy is a surgery done on the tummy using very narrow instruments. With this surgery, We check for any problems in the uterus ovaries or the nearby structures and correct them increase the chance of having a pregnancy.

It is tested with three different ways. First one is known as HSG or hysterosalphingography in which a dye is injected into the uterus and x-ray images taken to see whether the tubes are open or closed. The second test is SSG or Sonosalphingography in which instead of the dye,normal saline is injected into the uterus and using ultrasound, it is checked whether tubes are open or closed. The third test is by doing laparoscopy in which fluid is injected into the uterus and the confirmation of the tube is being open or closed is done by the instrument put into the tummy.

We need to see the reason for the sperm count being low. This is done by doing an examination of the genitalia, blood testing and if needed an ultrasound of the scrotum. Following this the plan can be devised as to how the counts can be improved. This can involve either medicines or surgery or combination of both.

There are many reasons due to which IVF can fail. It can be related to either the quality of the embryo or the problems in the uterus or problems during the embryo transfer procedure or very high stress level in the couples.

First and foremost, you need to ask your fertility specialist or fertility consultant the reason for the failed IVF. The primary thing to be done is to diagnose the reason why the IVF has failed and then correct that reason if possible. It is not very easy to reach to a reason and you might need certain specific testings for both husband and wife for the same. In case no reason has found out on the testings, there are a variety of treatments available to help such couples have a pregnancy and it will be best to consult your fertility specialist for the same.
